SUBJECT: [Changes in health insurance benefits]
Beginning next month, our health insurance carrier will increase the co-payment required on prescriptions. The co-payment for name-brand prescriptions will rise from [$10 to $12], and the co-payment for generic prescriptions will increase from [$4 to $6].
The monthly insurance premium for individuals also will increase by [15 percent] next month, while the family premium will rise by [14 percent. COMPANY], of course, will cover the premium increase for individuals; however, employees selecting the additional family coverage will need to cover the increase in that program.
Our human resources office has begun exploring other insurance alternatives and will keep us all posted on what it learns.
